Register

To register your child while living in the Manoa School Elementary District, complete the following forms and attach:

• ​Child's Birth Certificate
• Parent ID
• ​Proof of residence 

• Form 14 must be completed by doctor (Immunization Record, Physical, TB clearance) (PDF below)
